Liability Waiver and Release

I understand that martial arts training involves physical contact and exercise, and carries a risk of injury. I voluntarily choose to participate, or allow the student named above to participate, in classes, testing, and events at Dojo Forge.

1. Assumption of risk. I accept the risks that come with training, including strains, bruises, and other injuries, whether or not caused by the ordinary activities of the class.

2. Health. I confirm the student is in good health and I will tell the instructors about any medical condition that could affect their training.

3. Release. To the fullest extent the law allows, I release Dojo Forge, its owners, instructors, and staff from claims arising from participation, except for gross negligence or willful misconduct.

4. Emergency care. I authorize the staff to seek emergency medical care for the student if I cannot be reached.

5. Photos. Class photos may be shared in school newsletters and social posts. I can opt out at any time by telling the front desk.
